Diabetes: Medical Attention for Nephropathy

% of patients 18-75 years with Diabetes, who had a nephropathy screening test or evidence of nephropathy
QCDR-Measures-02

Controlling High Blood Pressure

% of patients 18-85 years with Hypertension diagnosis, whose Blood pressure was controlled
QCDR-Measures-03

Preventive Care and Screening: Body Mass Index (BMI) Screening and Follow-Up Plan

% of patients 18+ years with a BMI in the normal range(18.5 – 25 kg/m2) or outside normal range in last 12 months and have a follow-up plan

Preventive Care and Screening: Screening for High Blood Pressure and Follow-Up Documented

% of patients 18+ years, who were screened for high blood pressure AND a recommended follow-up plan is documented based on the current blood pressure (BP) reading as indicated

% of patients 18-75 years with Diabetes, who had hemoglobin A1c > 9.0% during the measurement period. *Lower score indicates better quality

Use of High-Risk Medications in the Elderly

% of patients 65+ years, who were ordered at least one high-risk medication.

Two rates are reported (lower score indicates better quality)

- Rate 1: % of patients who were ordered at least one high-risk medication

- Rate 2: % of patients who were ordered at least two of the same high-risk medications

Preventive Care and Screening: Tobacco Use: Screening and Cessation Intervention

% of patients 18+ years, who were screened for tobacco use one or more times within 24 months AND who received tobacco cessation intervention if identified as a tobacco user.

Three rates are reported:

- Rate 1: Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months

- Rate 2: Percentage of patients aged 18 years and older who were screened for tobacco use and identified as a tobacco user who received tobacco cessation intervention

- Rate 3: Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months AND who received tobacco cessation intervention if identified as a tobacco user

QCDR-Measures-09

Documentation of Current Medications in the Medical Record

% of visits for patients 18+ years for which the provider has documented list of current medications during the encounter. Medication list must include details (name, dosage, frequency, etc.) of all prescriptions, OTC, herbals, and nutritional supplements.
